Improving the “global use” of ultrasound for central venous access: a new supraclavicular scan by microconvex probe

چکیده
Background Ultrasound (US) is increasingly used as a “global” tool to assist all the steps of a central venous catheter insertion procedure: choice of vein/approach, needle guidance during venepucture, prevention of primary malposition, rule out of pleural complications. While approach choosing and needle guidance are now well standardized, US-based prevention of primary malposition and rule out of pleural complications are still a matter of debate. US based prevention of primary malposition of a central catheter relies on [1-4]: rule out of catheter misdirection in the superior vena cava (SVC) tributary veins (so called “negative assessment”) identification of the tip within the right atrium and/ or in the lower part of the SVC (so called “positive assessment”). Tip position may be evaluated:

Comparison Between Open and Ultrasonography Guided Venous Access Ports in Children with Malignancy
Background: Long-term central venous access is used in children for various reasons specially for delivering chemotherapy. Since vessels in children have smaller diameters, they are more prone to injury and complications such as thrombosis. Different methods are used for implantation of port-a-cath in children. We aimed to compare the complications of insertion of central venous access ports be...
متن کاملUltrasound Guidance for Central Venous Catheterization: A Step Further to Prevent Malposition of Central Venous Catheter before Radiographic Confirmation
Ultrasound (USG) guidance has long been used for guiding central venous catheterization. USG provides direct visualization of needle puncture through the skin into the vein. Most of the time USG guidance is just limited to puncturing of the vein and seeing guidewire entering the vein while malpositioning of catheter occurs after that which is seen later on while checking chest radiograph. Repos...
متن کاملAnatomical variations of the clavicle and main vascular structures in two pediatric patients : subclavicular vein cannulation with supraclavicular approach
Introduction Central venous catheterization (CVC) is implemented for volume resuscitation, hemodynamic monitoring, vasopressor administration, frequent blood sampling, parenteral nutritional support and the administration of long-term chemotherapy [1]. Generally, it is attempted using the internal jugular, subclavian or femoral veins. The subclavian vein access has been the recommended approach...
متن کاملComparing the anatomy of the intrathoracic systemic central venous system in Iranian pediatrics and adults based on chest CT scan and evaluation of rate of catheter malposition in children
Background: Central venous catheterization is a procedure that is being performed frequently especially in critical clinical settings. In such conditions, good knowledge of the surface anatomy of venous structures is vital to avoid possible complications which could result in life-threatening situations such as bleeding and pneumothorax. Considering the difference between venous anatomy of chil...
متن کاملUltrasound guidance for central venous catheter placement: results from the Central Line Emergency Access Registry Database.
BACKGROUND Ultrasound guidance of central venous catheter (CVC) insertion improves success rates and reduces complications and is recommended by several professional and regulatory organizations. METHODS This is a prospective observational study using data extracted from the Central Line Emergency Access Registry database, a multicenter online registry of CVC insertions from medical centers t...
